Tips for DBQ Completion
Any licensed medical provider (physician, NP, PA) can complete a DBQ — it does not have to be a VA provider.
Print the DBQ and bring it to your medical appointment so your provider can fill it out during your visit.
Make sure all sections of the form are completed. Incomplete DBQs may delay your claim.
Submit the completed DBQ with your disability claim or as supporting evidence for a pending claim.
